This is because the tattoo is placed under your skin, and so the needle comes into contact with your blood, and can pick up any blood borne illness, such as the HIV virus.

There are liner needles for fine detail work, and shader needles for filling in larger areas. The artist will usually switch needles between edging a design and filling it in, and also switch between ink colors.
